Participant Waiver

The undersigned, in consideration og being permitted to participate in the activities of the Traditional Karate Club of Wilmette LLC and Breakthrough Karate, and for other good and valuable consideration personally. does fully and forever release and discharge the Traditional Karate Club of Wilmette LLC and Breakthrough Karate and their respective officers, directors, members, managers, agents, teachers, instructors, successors, from any and all claims, injuries, damages, rights of action or causes of action of every kind and nature whatsoever, in any way or manner, resulting from any and all personal injuries sustained by me (including and without limitation injuries leading to my death) arising out of or relating to, or pertaining to my training and participation in karate and any activities associated therewith.

This waiver remains in full force and effect while participant is enrolled in classes or training seminars at the Traditional Karate Club of Wilmette LLC and Breakthrough Karate, unless and until patron advises the Traditional Karate Club of Wilmette LLC and Breakthrough Karate in writing he or she wishes to no longer be bound by it, and/or is no longer a participant at the Traditional Karate Club of Wilmette LLC and Breakthrough Karate.

I have read and I fully understand the contents of the foregoing waiver and release of liability, have executed this release and waiver freely and voluntarily, and agree to be fully bound by the foregoing terms and conditions.
